We are seeking an energetic and dynamic Service Coordinator (Case Manager/Social Worker). The Wood County Board of Developmental Disabilities (WCBDD) coordinates services for over 1,000 people with developmental disabilities in Wood County. Currently employs approximately 140 dedicated employees. Our mission is to support, empower, and inspire all people. WCBDD is looking for an experienced and dynamic Service Coordinator. The ideal candidate will be a highly self-motivated problem solver, who is a team player with time management and leadership skills. The Service Coordinator will manage a caseload of individuals, be responsible for writing the individual service plan and monitor the services to ensure health and safety. Essential Duties: Schedule and facilitate an Individual Service Plan (ISP)/ Individual Family Service Plan (IFSP) meetings using a person-centered philosophy. Develop and amend as needed the ISP/IFSP, including service authorizations for individuals based on assessed need and using the Interdisciplinary Team process. Be responsible for implementing, documenting and monitoring ISP data. Assess, coordinate and ensure implementation of services consistent with individual needs which are consented to in accordance with the Individual Service Plan. Provide direction of all services and behavioral programs. Ensure individual/guardian choice in the selection of providers. Collaborate with local school districts and Early Intervention providers. Provide information and assistance in securing services for eligible children and adults into educational, vocational, housing, and other community-based service. Document all service and support activities through Targeted Case Management. Share 24-hour on-call responsibilities with other Service Coordinators in crisis intervention and stabilization. Full-Time 80 hrs. We are seeking an energetic and dynamic Instructor (School Age) who has the skillset for implementation of an educational program for students. The Wood County Board of Developmental Disabilities (WCBDD) coordinates services for over 1,000 people with developmental disabilities in Wood County. Currently employs approximately 140 dedicated employees. Our mission is to support, empower, and inspire all people. The ideal candidate will be a highly self-motivated problem solver, who is a team player with time management and leadership skills. The Instructor develops and implements an educational program for school age students. a) Administers evaluative instruments to each child under his/her instruction to determine individual needs. b) Plans and develops an Individual Education Plan (IEP) based on multi-factored evaluation team report and evaluative procedure for each child under his/her instruction. c) Plans and implements units of instruction relevant to the persisting life problems of each child under his/her instruction, inclusive of but not limited to the following developmental domains: adaptive, aesthetic, cognitive, communication, sensorimotor, social-emotional and parent involvement. d) Plans and implements daily lesson plans designed to meet unit and Individual Education Plan objectives. e) Documents and reports evaluation results and student progress as scheduled. Fulfills all other obligations and duties as required for services, maintenance, routines and documentation processes including, but not limited to: a) Attends scheduled staff meetings. b) Attends/presides over meetings concerning students under his/her instruction (i.e. meetings, staffings, transdisciplinary meetings, parent conferences.) c) Maintains a cooperative relationship with instructors, specialists, ancillary professionals and administration in order to facilitate continuity of programming. d) Maintains a classroom environment conducive to learning and free of health and safety hazards. e) Administers medications and performs delegated nursing tasks as deemed necessary by a health care professional and delegated according to federal law, the Ohio Revised Code and rules promulgated under same and per collective bargaining agreement. f) Keeps accurate records of training, attendance, monies, and any other recorded material necessary for the maintenance of the program. g) Understands and implements emergency and evacuation procedures. h) Maintains classroom equipment/materials and makes recommendations for purchase. Follows Behavior Management Guidelines as appropriate and per the policy of the Board. a) Maintains accurate documentation records on students with behavioral concerns. b) Develops individual behavior programs as appropriate. c) Implements approved behavior programs for individual students, evaluating and revising as needed. Maintains a close cooperative relationship with the family of each child under his/her instruction in order to best facilitate and implement the Individual Education Plan. Provides work direction and training to instructor assistants, aides, volunteers, observers and students in training as necessary for classroom and extracurricular activities. Minimum Qualification Standards:
1. Must be able to move individuals who are receiving services including adults and children in a safe manner. Must be able to lift, carry and move office supplies, materials, and equipment required for day-to-day operations.
2. If required by the position you must maintain a current driver’s license in your residing state for the purpose of transporting individuals who are receiving services and/or operation of a Board owned vehicle.

Conditions of Employment
1. Successful completion of general orientation.
2. Receipt of six acceptable background required by ORC 3319.39 & OAC 5123:2-2-02.
3. Must obtain and maintain any training, licenses, registration, or certification mandated by the Board and the State of Ohio.
4. Must have a medical examination and a one-step PPD test to determine fitness for duty prior to providing direct care service or within 30 days of employment whichever comes first.
5. Possess/maintain an acceptable motor vehicle record (if required for position) per board guidelines. NOTE: Use of personal vehicle for transporting consumers requires proof of liability insurance.
6. Receipt of acceptable criminal background report from Bureau of Criminal Identification (BCI) and county sheriff ‘s office of permanent address before starting position as required by ORC 3319.39 & OAC 5123:2-2-02.
